• We strongly recommend bloodwork on all pets prior to anesthesia. The liver and kidneys are the primary organs responsible for removing the drugs we use for anesthesia; therefore, it is very important for us to know about these organs. If we find any unusual information in the bloodwork, we can discuss these findings with you and choose the safest, specific drug protocol for your pet. We may also decide that it is best to postpone procedures while addressing any problems we find. Normal bloodwork results do not full guarantee that we may not have any unforeseen problems we find, but they do help us to be sure your pet is at low risk of complications. There are also useful for baseline values for future reference.

  • Anesthesia will cause your pet's blood pressure to decrease. This will slow the blood circulation to the organs. IV fluids are used to help maintain adequate blood pressure and blood circulation to the vital organs.

    Please check to authorize.
  • ***We very strongly recommend Rabies and Distemper vaccines for your pet.

***If your pet has fleas, for the safety of our hospitalized clients, we will treat your pet while it is in the hospital. There will be a $7.00 fee if we need to treat him.

As the owner agent (must be 18 years or older) of the above mentioned animal, I authorize the doctors and staff of Eugene Animal Hospital to anesthetize my pet and perform therapeutic, surgical or emergency procedures deemed necessary in the doctor's professional judgement. I understand that there are inherent risks with anesthesia and although modern veterinary anesthetics are generally considered safe, problems may still occur. The Hospital and staff will NOT be held liable for any problems that develop provided reasonable care and precautions are followed. I assume full responsibility for all charges incurred for the care or my animal. I agree to pay for any service fees and collection fees. If I neglect to pick up my pet within 5 days of the date below and do not notify you within that time frame, you may assume the pet is abandoned and are hereby authorized to dispose of the pet as you deem best and/or necessary.
